Developmental Pathways has a fantastic opportunity for a Case Management Specialist who will be responsible for providing professional level administrative support to the Case Management department. Case Management Specialists develop and maintain systems of organization, distribution, and communication, as well as, provide additional case management support to the Case Managers, as appropriate.

We have two office locations in the Denver, CO Metro area - one in Aurora, CO and one in Englewood, CO (just south of the Denver Tech Center). The Case Management Specialist position location will be dependent upon the team needs at each location. We’re committed to a great work-life balance here, so our offices are open Monday - Thursday and are closed on Fridays. This is a full-time 36-hr/week position working Mondays - Thursdays.

More about what you’ll get to do as our Case Management Specialist

  • Develop, distribute, and manage service plan (SP) and billing data for service providers
  • Support and resolve billing issues
  • Research participants’ service utilization and previously-provided services and supports and follow up
  • Analyze work completed to identify and report on trends
  • Track problems and manage workflows effectively
  • Ensure timely initiation and implementation of the SP process
  • Ensure timely completion of documentation needed for the provision of services and funding
  • Perform and document targeted case management activities within established timelines
  • Complete annual SPs and revisions for the plans of individuals in service
  • Check/review all annual SPs and SP revisions to ensure all requirements are met and all data is accurate
  • Ensure the accuracy and timely entry and distribution of written SPs and addendums
  • Enter SP data into state databases, as assigned
  • Calculate all support levels and communicates changes to providers, case managers, and other pertinent staff
  • Review/approve/manage Home Health PARs, as assigned
  • Coordinate/manage request for proposal process as assigned
  • Help obtain signed medical documentation from medical staff, as assigned
  • Develop/maintain tracking systems for various case management and program functions; create reports from tracking
  • Develop and use formats for information sharing (letters, memos, list, spreadsheets, etc.)
  • Create calendars and/or appointments for meetings and events
  • Enter data accurately and punctually; generates database reports, as appropriate
  • Establish and maintain well organized filing/e-filing systems, including financial records
