The Care Manager is responsible for the implementation and facilitation the Momentum Program. The Momentum Program will support both Colorado Mental Health Institute Social Work departments and acute hospitals in the re-entry of difficult to place individuals back into their home communities. The Momentum team will facilitate community reintegration, which includes managing the wraparound services, which are not otherwise supported with insurance or other funding sources for persons transitioning from the two state mental health institutes (MHIs) or an acute hospital. This position maintains responsibility for collaborating with the MHIs, acute hospitals, Community Mental Health Centers (CMHCs) and other providers throughout the State of Colorado.

Areas of Responsibility

Essential Duties

  •  Responsible for increasing service capacity to more seamlessly address the service needs of those individuals who have been committed, certified, who or are voluntarily receiving emergency stabilization behavioral health treatment services.
  • Carry out Care Manager responsibilities, in accordance with company policies, applicable laws, and the Momentum contract to assist clients and communities in managing the transition from hospitalization to less restrictive alternatives, employing a wraparound services approach. 
  • Must participate at minimum bi- monthly supervision, weekly supervision is at the discretion of the Program Manager. 
  • Develop, monitor, update, and follow policies and procedures to address Momentum issues pursuant to the contract.
  • The Care Managers will assist clients in accessing services, assist clients and communities in coordinating various services and funding sources, identifying and strengthening formal and informal community resources, and mobilizing helping networks
  • Identify and report to the manager on team activities and issues that may positively and/or adversely influences the delivery of services under the contract.
  • Coordinate Continuity of Care with CMHCs, other local providers, communities, and Regional Accountable Entities (RAE).
  • Coordinate with all 17 CMHCs, and particularly with Assertive Community Treatment (ACT) Teams.
  • Work with the Division of Housing in the Colorado Department of Local Affairs and Public Housing Agencies to apply for appropriate housing vouchers when needed, and support individuals in locating, applying for, and attaining a housing rental.
  • Coordinate with the individual’s RAE (if applicable) for needed behavioral health and physical health care needs.
  • Work with the Social Workers, Benefits Coordinators and CMHC to ensure that clients overcome barriers to applying for eligible benefits prior to discharge from MHI or acute hospital or in the community when necessary.
  • Work with client and interested parties to identify and purchase goods/services not covered by Medicaid or another payer source that are specific to individualized needs.
  • Serve as communication conduit between contracted providers and other interested parties (CMHC)
  • Coordinate with regional Crisis Stabilization Units, nursing homes, and ALR facilities
  • Provide for and support the successful community engagement of persons referred by the MHIs, acute hospitals and CMHCs to the Continuity of Care program, throughout the State of Colorado, beginning, at minimum 30 days prior to their discharge from the institutes and for up to 60 days after their return to their communities. Additional time may be added depending on the transitional needs of the client. For acute hospital and CMHC referrals the team will engage as quickly as possible, upon receiving the referral.
  • Meet with client as soon as possible to review the Program (Momentum Team), obtain input for plan, and have client sign appropriate Release of Information (ROI) forms. 
  • Ensure a member of Momentum Team will accompany clients to their first appointment at the CMHC or other provider following discharge. 
  • Collaborate with Momentum Peer Bridgers in relation to care plans, coordinating services and treatment correspondence.
  • Complete documentation as required by OBH and/or RMHS; ensuring that patient contacts and collateral contacts are entered into the system.

Driving Requirements

  • Valid drivers license
  • Proof of motor vehicle insurance
  • Personal vehicle in good operating condition for use during work, including transporting clients
  • No major violations in the past three years.
  • No more than two moving violations in the past three years
  • Ability to meet and maintain agency driving requirements and operate agency vehicles
  • Momentum and TSP Staff must include a Business Use policy on their personal automobile insurance.
  • Drivers must upload proof of ongoing auto insurance every 6 months into Paycom/Relias.
  • Drivers must carry Bodily Injury Liability Limits of $300,000 each person/$300,000 each occurrence. Additional liability limits are recommended.
  • Any Accidents and/or damages while using a personal vehicle for work purposes shall be the sole responsibility of the employee. RMHS is not responsible for any damage incurred to the employee’s personal property.

 PREFERRED QUALIFICATIONS

  • Eligible within one year or currently hold a Colorado License, without sanctions, required in a behavioral health discipline such as LCSW, LMFT, LPC, LAC, or licensed psychologist
  • 5 years of mental health direct care experience.
  • Graduate degree in social work, social services, or related field
Requirements/Qualifications
  • Bachelor’s Degree in Social Work or a related field.
  • 2-5 years of mental health direct care experience  
  • Ability to complete required training as required by the State of Colorado.
