Job Description: 

Transition Specialists provide intensive case management services to individuals with behavioral health conditions, including substance use disorders. Support successful transitions into the community. Coordinate services with community-based systems. Insure provision of all wraparound services. Monitoring of services provided to clients. This position maintains responsibility for collaborating with acute hospitals, detox facilities, Community Mental Health Centers (CMHCs) and other providers throughout the State of Colorado.

This is a remote, community-based position.

Areas of Responsibility

Essential Duties

  • Assess client needs and assist in accessing services, coordinating care and identifying funding sources, including formal and informal community resources, and mobilizing helping networks.
  • Coordinate continuity of care with CMHCs, and other treatment providers.
  • Work with various agencies to secure housing, benefits, healthcare providers or any needed service.
  • Coordinate with the individuals RAE (if applicable) for needed behavioral health and physical health care needs.
  • Collaboratively creates and monitors care plans for individuals assigned to caseload within contract timelines
  • Initiates and writes referrals for services
  • Provides assistance with service and support placements
  • Communicates effectively with clients, families, agency staff and others
  • Reviews, responds, and provides follow up regarding any client paperwork
  • Completes face-to-face visits with individuals while inpatient, in the community and at their homes
    Serves as a mediator and helps to resolve conflict as it relates to the individuals service plan
    Advocates for the best interest of the client
  • Provide for and support the successful community engagement of persons referred by the acute hospitals, withdrawal management, and CMHCs to the Continuity of Care program, throughout the State of Colorado,
  • Accompany and/or transport clients to their appointments as necessary.
  • Collaborate with Peers in relation to care plans, coordinating services and treatment correspondence.
  • Performs other duties as assigned

Preferred Qualifications

  1. Eligible for or currently hold a Colorado License, without sanctions, required in a behavioral health discipline such as LCSW, LMFT, LPC, LAC, or licensed psychologist
  2. Fluent in written and spoken Spanish
  3. 5 years of mental health/Substance Abuse direct care experience.
  4. Graduate degree in social work, social services, or related field

  • Sit, stand and walk for reasonable periods of time.
  • Maintains prompt and regular attendance.
  • Performs related work as assigned.
  • Ability to drive personal or company vehicle

Driving Requirements

  • Valid drivers license
  • Proof of motor vehicle insurance
  • Personal vehicle in good operating condition for use during work
  • No major violations in the past three years
  • All TSP Staff must include a Business Use policy on their personal automobile insurance
  • All TSP Drivers must upload proof of ongoing auto insurance prior to expiration date in Paycom
  • All TSP Drivers must carry Bodily Injury Liability Limits of $300,000 each person/$300,000 each occurrence. Additional liability limits are recommended
  • Any Accidents and/or damages while using a personal vehicle for work purposes shall be the sole responsibility of the employee. RMHS is not responsible for any damage incurred to the employee’s personal property.

Requirements/Qualifications
  • Bachelors degree in Human Services (or related field) and 2-5 years working with people in Crisis Services/Substance Abuse Disorders.
  • Ability to complete required training as required by the State of Colorado.
  • Ability to receive, process, and maintain information through oral and/or written communication effectively
