Organizational Mission: Project Angel Heart improves health and well-being for people with life-threatening illnesses by preparing and delivering medically tailored meals throughout Metro-Denver, Boulder, and El Paso County and promoting the power of food as medicine.

Position Summary:  Coordinates with clients and referrers on applications, intakes, and recertification of meal service through core home-delivered meal program; collects, reports, and analyzes data and projections for grants; researches and coordinates logistics of outreach opportunities as necessary.

Amount of Time and Status: Full time, exempt, Monday-Friday

Reports to: Director of Client Services

Areas of Responsibility
  • Assists with all intakes and processes for core home-delivered meal program (including creating and maintaining how-to manuals & staff training)
  • Completes intakes and outcomes for the Meals for Care Transitions, as necessary
  • Distributes and tracks receipt of required documentation for clients, as related to grant funding (i.e. proof of income), and assists with data entry for reporting
  • Maintains client database by tracking changes, making updates, trouble-shooting, report/query building, etc.
  • Maintains and updates required client reporting data as mandated by grant funding, as necessary
  • Runs weekly reports to ensure clean data entry and follows-up with staff as needed
  • Assists with training and managing volunteers completing data entry
  • Assists with recertification processes through data entry, tracking changes, updating records, and oversees all necessary follow-up
  • Oversees all administrative tasks (ie: prepacked mailings, stamped envelopes, client related flyers, etc.)
  • Oversees annual client appeal including generation of list, printing, mailing, and necessary follow-up
  • Serves as a primary contact for all client matters
  • Tracks and reports data for development requests, including service goals, benchmarks, and various population trends; projects service numbers for upcoming proposals; coordinates with other staff to meet all grant-related goals and site visit compliance
  • Supports all department outreach efforts by researching opportunities and coordinating details for the Director of Client Services as well as networking and attending specified meetings
  • Completes annual evaluation for core program, as necessary
  • Serves as primary contact for Spanish related matters, including intakes, translations, and outreach
  • Oversees the maintenance and filing of client records
  • Identifies clients appropriate for marketing and philanthropic purposes
  • Stays informed on research as it relates to disease and nutrition through periodical education, professional networking, and conference attendance
  • Other duties as assigned

Working Environment/Physical Activities

Work environment is an office setting.  Physical activities include sitting at desk and working on a keyboard and computer several hours a day; conducting business over the telephone; using a fax and copy machine.  Also requires driving a vehicle on a regular basis, moving agency materials up to 45 pounds, and moving materials up to 45 pounds in and out of vehicle.  This position requires the ability to enunciate clearly and interface professionally with the public.

  • Undergraduate degree from an accredited institution in a human services field or equivalent work experience
  • Minimum of two years’ experience in a direct human services environment
  • Personal or professional experience with people living with life-threatening illness and/or mental illness
  • Experience working with Microsoft Office Suite, and a CRM database (specifically ClientTrack) preferred
  • Comfort with reviewing, analyzing, and projecting data and trends
  • Ability to create and consistently maintain information organizational systems
  • Ability to conceptualize multi-step processes, effectively coordinate with co-workers, and independently pursue projects to completion
  • Fluency in Spanish required
  • Exceptional attention to detail and ability to correct data with confidence
  • Excellent verbal and written communication and interpersonal skills
  • Ability to produce quality work and meet deadlines while multi-tasking
  • Demonstrates approach with clients that accentuates client-centered service; ability to relay messages from/to clients accurately
  • Willingness and ability to maintain client confidentiality
  • Willingness and ability to work with diverse populations
  • Proven history of working as a team member in a fast-paced environment
  • Ability to prioritize projects and client needs as relating to service delivery
  • Excellent discretion in judgment and ethics relating to client needs and service
  • Valid Colorado driver’s license and own transportation preferred

Pay will be commensurate with experience. Salary range is 41K-42K.
