Job Description: 

The Health Partnership’s mission is to compassionately connect people to health and wellbeing resources. The organization serves as a resource hub for the communities of the five-county region in northwestern Colorado in two unique ways. Through our Community Impact work, we focus on the social and economic factors that influence the health of our communities with programs that connect community and clinic resources more effectively. We also work with partners across sectors to coordinate and collaborate on efforts to address population-wide health issues and the systems that influence them through a newly launched Yampa Valley Wellbeing Consortium. Through our Integrated Care Coordination team, we connect individuals facing a complex set of barriers, such as poverty, social isolation and health issues, with community resources to achieve their health and well-being goals.

Areas of Responsibility

The VISTA will be part of the Community Impact team and will specifically work on the Yampa Valley Wellbeing Consortium, a 40+ member group that is facilitated by The Health Partnership to address shared population health issues across Moffat and Routt counties. The Community Impact Coordinator will support Consortium action team action plan virtual meetings; collect, update and communicate quantitative and qualitative data housed in the YVWC data dashboard; and contribute to social determinants of health training, education for partner organizations and community members.

Requirements/Qualifications
  • Car recommended
  • Be 18 years or older (no upper age limit)
  • Hold one of the following citizenship or legal residency statuses: US citizen, US National, Lawful Permanent Resident (i.e. Green Card status), and persons legally residing within a state.
Compensation/Benefits
Compensation: 

Compensation is determined by Americorps (by county). The range for AmeriCorps VISTA is $13,362 - $20,584 annually. 

Benefits: 

Workspace, phone, computer,  Choice of Education Award or End of Service Stipend,  Relocation Allowance,  Living Allowance.
