Direct Supervisor: Housing Services Director

Program: Emergency Shelter and Outreach Programs

 

Job Location: Brighton, Almost Home, Inc. main office.

Work Schedule: Monday - Friday - 8:00 a.m. – 5:00 p.m.

 

Summary of Responsibilities: 

The Housing Navigator provides case management and essential services to individuals and families experiencing homelessness in Adams County and Southern Weld County. The Housing Navigator will provide case management and re-housing assistance to households living in Almost Home, Inc.’s Emergency Shelter and will provide outreach support and case management to households experiencing literal homelessness in the community.

 

The Housing Navigator will provide individualized client support by helping each client develop a Housing Stabilization Plan to address any barriers to obtaining and maintaining permanent housing. The Housing Navigator assists Emergency Shelter clients with re-housing into permanent housing. The Housing Navigator will also perform community outreach to business owners, realtors, landlords, housing developers and other service providers to build strong relationships and identify new and existing opportunities to better assist clients in accessing resources, employment, supportive services, and housing opportunities in the community.

 

The Housing Navigator will also provide outreach and supportive case management to individuals and families experiencing literal homelessness. The Housing Navigator will provide supportive case management services in coordination with the Severe Weather Activation Plan to ensure that households experiencing literal homelessness can gain access to survival resources and case management services. Case Management will include but is not limited to: comprehensive, continual assessment of strengths, needs, assist clients in navigating resources in other separate public agencies in order to qualify for support services, and will link the households to the many private sector agencies and community organizations that offer specific support services that address Social Determinants of Health. 

 

The Housing Navigator is responsible for developing Housing Stabilization Plans with clients to help meet Almost Home, Inc.’s overall mission to promote self-sufficiency and provide assistance to those experiencing housing instability.

Areas of Responsibility

Direct Care Duties include:

  • Responsible for assessing the needs of clients experiencing challenges related to housing insecurity and linking them with internal and external resources in the community; Establish and develop Housing Stabilization Plans for all families residing the Almost Home, Inc. Emergency Shelter and provide those families with supportive services to obtain permanent housing. Supportive services include, but are not limited budgeting, connection to workforce development, identifying new appropriate housing placements, and direct emergency assistance in the form of rental and/or utility financial assistance; Assist families to establish eligibility for subsidized housing;
  • Perform outreach services, contacting homeless persons in all places where they congregate in the geographic areas covered under our contracts. Heavy emphasis on engaging with people living in encampments, parks, beaches, abandoned buildings and other locations unfit for human habitation. Respond to community requests for street outreach intervention; Coordinate outreach with regional agencies, governmental organizations and homeless navigators at the municipal level;
  • Distribute engagement items like bus passes, hygiene products, gloves, coats, hats, etc. to provide relief to people experiencing unsheltered homelessness;
  • Refer clients to the SWAP and Respite Housing program for hotel/motel vouchers; assist with SWAP intakes during periods of inclement weather; coordinate case management services with SWAP and Respite Housing program.
  • Assist clients with procuring necessary documents and services such as identification card, birth certificate, social security income, disability income;
  • Conduct VI-SPDAT assessment on all individuals and families to connect with other homelessness resources in the region;
  • Provide referrals to any needed services including health, mental health, benefits, education and/or vocational training;
  • Serve as an advocate or liaison for households with landlords and/or other community organizations;
  • Provide ongoing, short-term case management to families following housing stabilization to ensure success;
  • Document progress toward desired outcomes – housing, health and income stability. Maintain client related data tracking systems, including case notes and complete HMIS entries. Client data entry as required to meet contractual obligations. Data entry into the state HMIS data management system;
  • Ensure timely and effective flow of communication between internal departments to ensure the delivery of quality services to the client;
  • Other duties as assigned

Knowledge and Skills

  • Passion for ending homelessness.
  • The ability to maintain a non-judgmental attitude and a display of unconditional positive regard when working with persons experiencing homelessness.
  • Knowledge of and familiarity with Trauma Informed, Harm Reduction and Housing First models of service delivery. 
  • Ability to network and build relationships; locate and develop housing resource contacts.
Requirements/Qualifications
  • 1. Bachelor’s Degree in Human Services or comparable combination of education/work related experience required;
  • 2. Experience in a social services setting with working knowledge of case management systems and planning techniques;
  • 3. Experience working with homeless individuals and families is strongly preferred;
  • 4. Must be proficient in the following computer applications--Microsoft Word, Excel and Outlook;
  • 5. Experience with data entry and client documentation. HMIS experience preferred.
  • 6. Must have and maintain a valid CO driver’s license. Must have own transportation to conduct job related travel.
  • 7. Bilingual preferred;

Compensation: 

Salary Range: $22.00 per hour

Position Type: Full Time, non-exempt
