Service Coordinator/Case Manager - Early Intervention (Children Birth-3) 

Developmental Disabilities Resource Center of Lakewood, CO is seeking to hire a full-time Service Coordinator/Case Manager - Early Intervention (Children Birth-3) to provide resource coordination to children (birth-3) who are enrolled in early intervention and family support services program.

Areas of Responsibility

A DAY IN THE LIFE AS A SERVICE COORDINATOR/CASE MANAGER - Early Intervention (Children Birth-3)

As a Service Coordinator/Case Manager - Early Intervention (Children Birth-3), you are responsible for working with families to identify the needs of their child. In addition you develop individual family support plans outlining services they will receive and communicate information about the referral process. 

 

Requirements/Qualifications
  • Bachelor's Degree in human services or related field strongly preferred, or a combination of education and experience in the field of Intellectual and Developmental Disabilities or Early Childhood is considered.
