Course Description:
Are the same incidents occurring over and over again? Discover how investigations can be used as a learning tool to identify causal factors of an incident.
During this program, participants will:
- Outline how to use incident investigations as a learning tool to improve the system.
- Discuss the importance of being open to multiple causal factors.
- Define human error as a symptom not a cause of incidents.

Jon VonderHaar, Human Performance Specialist, Pinnacol Assurance
Jon VonderHaar joined Pinnacol Assurance as a safety consultant in 2018. VonderHaar works closely with Pinnacol’s customers in the healthcare field, helping them achieve improvements in workplace health and safety. He enjoys helping others understand the reasons behind his recommendations, empowering them to take ownership of their safety practices and well-being. VonderHaar’s extensive background in physical abilities testing, work diagnostics, occupational rehabilitation, work conditioning, safety, ergonomics and therapeutic exercise programs is a strong foundation for the services he provides.
VonderHaar’s previous experience includes roles as the regional coordinator for physical abilities testing for U.S. Healthworks, director of work diagnostics for U.S. Healthworks Bakersfield Medical Center, director of occupational rehabilitation and testing services for Central Valley Occupational Medical Group, and coordinator of the cardiac rehabilitation program at the Veterans Administration Medical Center in Dayton, Ohio. For three years prior to joining Pinnacol, he worked as an ergonomics consultant for California’s State Compensation Insurance Fund. VonderHaar earned a bachelor’s degree in exercise science with a concentration in kinesiotherapy and a master’s in exercise science with a concentration in biomechanics, both from the University of Toledo. He has also earned the Certified Safety Professional designation.
